A spokesman for Sandwell Children’s Safeguarding Partnership said: “Our thoughts are with everyone who knew Shay.

“The Partnership has commissioned an independent review of local child protection practice (LCSPR) in line with our statutory duties to consider whether there are any learning opportunities arising from this tragic event.”

The date when the report will be published has not yet been announced.

“There is no community yet, but the process is ongoing,” a spokesman said.

“All partners will participate in an independent review, which will be published once it is complete.”
